The Sunday Telegraph have reported that the terms are nearly agreed for the Colombian to move to Stamford Bridge next term, although he will be taking a cut in his £265,000 per week wage.
Falcao had previously played alongside Diego Costa at Atletico Madrid. In addition, Jose Mourinho and Falcao share the same agent, Jorge Mendes.
The 29-year-old Falcao endured a frustrating season on loan at Manchester United last season, scoring only four goals for the Old Trafford club.
Falcao s currently away at the Copa America in Chile.
While Falcao could be on his way to Chelsea, one player who looks set to leave Stamford Bridge is Loic Remy.
Remy, who had fallen down the pecking order behind Costa and Didier Drogba last season, has been linked with an £8m move to West Ham and Crystal Palace.
