Th 6ft 4in 20-year-old centre-back is rated as one of the top prospects in Major League Soccer and has already been capped by Jurgen Klinsmann.
According to ESPN, Miazaga is already on his way to England with a move set to be announced in the next 24 hours - other reports state Miazga will cost the Blues around £3.5million as he has a less than a year left to run on his current deal.
Miazga, who was also linked with Liecester City and Stoke City, has a Polish passport and will therefore not have to obtain a work permit.
England centre-back Gary Cahill has been linked with a move out of Stamford Bridge, reported to be concerned about his place in the national side ahead of Euro 2016.
Interim manager Guus Hiddink has made it clear that he prefers Kurt Zouma to the experienced Cahill, starting the Frenchman in all but one of the eight games since he took over.
Cahill, 30, has not started in the Premier League since Boxing Day, although he is arguably considered a first-choice for Roy Hodgson's England.
It is unclear which clubs may be interested in Cahill at this stage.
