The centre-back played every minute in the Premier League this season as he led his team to the fourth title of his career.
With former England team-mates Frank Lampard and Steven Gerrard set to move to the US this summer, the 34-year-old has admitted that he may consider moving stateside, but would like to stay in west London.
"I would love to finish my career at Chelsea and maybe there is a time when I look to go to play in America or whatever might be best for my family at the time," Terry told TalkSPORT.
"Hopefully I have a couple of years left at least. I always thought I could get to age of 35 and still (be) playing and I am getting close to that now."
This season saw the first trophies of Mourinho's second spell won, with the Blues also winning the Capital One Cup.
The Portuguese won two league titles in his first spell, from June 2004 to September 2007, with the core of the team staying together for the best part of a decade and also winning the 2012 Champions League.
Terry believes the current squad, and the youth players coming through the Chelsea ranks, is capable of surpassing those achievements.
He added: "Looking at this squad now, I think this squad has the potential to do a lot more than we did in the past, well beyond my playing days."
