N'Golo Kante opened the scoring after 27 minutes before Eden Hazard doubled the hosts' lead with his strike on 38 minutes.
But Spurs hit back after the break, with Fernando Llorente heading home five minutes after the restart to bring the score level across the two legs.
Both sides pushed for the winner but, with the score tied at 2-2 come the final whistle, this second-leg clash was taken straight to penalties where Chelsea went on to secure victory.
Maurizio Sarri's men now face Manchester City in next month's final at Wembley.
