Chelsea manager Jose Mourinho has made just one change for the title decider against Crystal Palace.
If the Blues beat the Eagles they will win the fifth title in their history and their first since Carlo Ancelotti won the Double in 2010.
Thibaut Courtois coming in for Petr Cech is the only change to the Chelsea team that produced an excellent second-half display against Leicester in midweek.
For Crystal Palace, Alan Pardew has made four changes to the team that lost to Hull City 2-0 last week.
Adrian Mariappa, Jordon Mutch, Joel Ward and Joe Ledley come in for the Eagles.
