The city pleasure cruise was targeted as it passed underneath a road crossing on the River Spree.
Berlin Fire Department said the incident caused passengers on the top deck to jump up and hit their heads on the low-level Jannowitz bridge.
“As a pleasure boat on the Spree passed under a bridge, a person urinated from above on the ship,” the fire department tweeted.
“Several people jumped up and were injured while passing through. Four people were taken to hospital by ambulance.”
The injured passengers, including three women, aged 38, 39, and 48, and a 54-year-old man, were discharged after receiving treatment to minor.
Sixteen rescue workers attended the scene on Friday.
Police described the incident as “abnormal” and said the suspect had not been identified.
