Emergency services were called to the collision on the A30 near Cheriton Bishop, Devon, at about 7.40am on Wednesday.
Police say the HGV overturned onto its side and two people were believed to be seriously hurt.
Gary Bayley, who shared video footage of an air ambulance circling above the scene on Twitter, said a “truck through central reservation has hit oncoming traffic”.
Devon and Cornwall Police said in a statement: “Both carriageways are closed and are likely to remain shut for some time.
“Diversions are in place westbound at Fingle Glen/Pathfinder and eastbound at Woodleigh junction.
“Police are advising motorists to avoid the area if possible for the time being.”
