The actor's family confirmed he died of cancer on Wednesday (2 January).
Perkins played the original Scott Robinson when the show launched in 1985 on Australia's Channel Seven.
After the series moved to Network Ten in 1986, Jason Donovan took over the role after Perkins was fired for his behaviour on set, including lateness.
Perkins later returned to the show, in 2013 and 2014, playing a new character named Marty Kranic. Prior to his return, he had not had an acting role in 19 years.
His Neighbours co-star David Clencie, who was also a close friend, told the Herald Sun: "I am so sad, really devastated to lose my mate. We had this incredible bond. We were mates to the very end."
Perkins also appeared in multiple Australian dramas, including Home and Away, A Country Practice, Prisoner and The Sullivans.
