Beaten to death by stepfather, despite 30 calls from neighbours to social services. The inquiry said: "The system failed her. Time and time again, we have had to refer to a failure of communication".

1984: JASMINE BECKFORD, 4

Starved and battered to death by her stepfather, who had been convicted of assaulting her younger sister. Seen by a social worker only once in 10 months. Official report said her social worker was at fault.

1985: HEIDI KOSEDA, 5

Died of starvation and found in a cupboard two months later. Her mother admitted man-slaughter and the boyfriend was jailed for murder. The NSPCC did not investigate reports of child abuse.

1986: KIMBERLEY CARLILE, 4

Starved and beaten to death by her stepfather. An inquiry found her death was avoidable and found four key social work and health staff failed to apply the necessary skill, judgement and care.

1992: TONI DALES, 3

Died in 1992 from head injuries after being thrown on a wall by her mother's lover. A report cited failure by social services, health, police, education and probation services.

1992: LEANNE WHITE, 3

Beaten to death by her stepfather after suffering 107 external injuries. An inquiry concluded her death could have been prevented if social services had responded to reports that she was at risk.

1994: RIKKI NEAVE, 6

Found strangled. Rikki had been on the council's at-risk register. His drug addict mother, cleared of murder but jailed for seven years for cruelty, had asked social workers to take her son. Report described "a legacy of incompetence" among social services chiefs.

1997: LAUREN CREED, 5

Died after being thrown downstairs and jumped on by her stepfather. An inquiry by the Norfolk Area Child Protection Committee showedsystem-wide failures - from probation officers, social services, the police and local health authorities.

2000: LAUREN WRIGHT, 6

Died from a blow to her stomach inflicted by her stepmother, who was found guilty of manslaughter, as was the girl's father. An inquiry found that inter-agency co-ordination was "ineffective" and social workers had not acted with "due urgency".

2000: VICTORIA CLIMBIE, 8

Starved, beaten and trussed in a bin bag. Her great-aunt and boyfriend were jailed for life for murder. Victoria had come into contact with four sets of social services, three housing departments, two police protection teams, two hospitals and an NSPCC family centre.

2002: AINLEE WALKER, 2

Died at the hands of her parents. Had 64 separate injuries, including cigarette burns. Her parents were jailed for a total of 22 years. A report said: "Health agencies, the police and housing all had serious concerns, but none of this was passed to social services."

2003: NATALIE MILLS, 15

Died after being assaulted by her boyfriend. Social services were jointly responsible for her. Natalie's case was not allocated to a specific social worker because of shortages. A review criticised "significant actions or non-actions" by social services.
